In 2010, 47.8 percent of eligible plan members took the assessment – an impressive figure, but not enough to put us over the top. Your participation can make a difference for all of us. Think what a 2% reduction in health care costs could make possible in your congregation – then multiply that across our entire Synod.

• If you are a plan member or spouse, please take 30 minutes to complete the assessment by April 30.
• If you're a congregational leader, please encourage your church's plan members to take the assessment.

The confidential assessment is quick and easy, and once you complete it you will receive a $150 wellness account credit. After you take the health assessment, you can visit the Board of Pensions/Mayo Clinic site to participate in other activities and earn an additional $300 wellness credit.
